Community Health Workers and Home Visiting professionals share a common mission: supporting families during their most critical years. The Home Visiting Community Advisory Board (HV CAB) offers CHWs an opportunity to influence statewide strategies for early childhood services and strengthen the continuum of care for Delaware families.
Our Vision
Pregnant women and parenting families in Delaware will have access to voluntary, high-quality early childhood home visiting services as part of a comprehensive system providing support, encouragement, and resources so children and families can thrive.
Our Mission
Ensure a collaborative approach to home visiting, advancing equitable and diverse research-informed programs supporting individuals from prenatal through age 5.
The CHW-Home Visiting Connection
Both CHWs and Home Visitors work directly with families, providing support, resources, and system navigation. HV CAB creates the collaborative infrastructure that makes your work more effective by reducing duplication, aligning resources, and advancing shared objectives for families and children from prenatal through age 5.
How CHWs Benefit from HV CAB Participation:
- Cross-sector collaboration with key stakeholders including the Division of Public Health (DPH)
- Department of Services for Children, Youth and Their Families (DSCYF), Delaware Department of Education (DOE), United Way’s Help Me Grow (HMG), and healthcare systems
- Streamlined referral pathways between home visiting programs, healthcare providers, and community resources
- Input on policy and service delivery that affects the families you serve
- Access to a comprehensive network of early childhood education and family support services
- Coordination that prevents gaps in services and ensures families receive appropriate support
