Careers and Volunteers
CHWs are a valued part of the health care team. They serve as frontline liaisons, guiding individuals and families through the health, social, and community services systems to achieve better health and well-being.
CHWs support communities throughout Delaware, working with hospitals, Federally Qualified Health Clinics, the Division of Public Health, community organizations, and managed care organizations. CHWs are part of an equity-focused public health approach to support individuals and communities based on their health-related social needs.
Already a CHW? Now you can get certified. A certification provides all CHWs with the ability to provide consistent care as well as a path to advance their career and education.

Impact Life (Kent County)
Position: Certified Community Health Worker
Rural Health Hub team in Dover, Delaware. Serves as a primary bridge between participants, healthcare systems, and community resources, supporting individuals and families throughout Kent County and surrounding rural communities who may experience barriers to traditional healthcare.
ChristianaCare (Kent and Sussex Counties)
Position: Community Educator (2 openings)
Home Visiting Community Health Worker Team. One opening based in Kent County and one in Sussex County, supporting outreach, education, and care coordination for pregnant and young families with children under the age of 4. Bilingual candidates are strongly preferred.
ChristianaCare (New Castle County)
Position: Community Health Worker - Women’s Health
Women’s Health Community Health Worker Team. Serves pregnant patients through their pregnancy and postpartum care journeys.
