Our Team
The Community Health Workers Association of Delaware (CHWAD) is a membership of people working together to improve the health and health outcomes of Delawareans. Collectively, we represent over 35 diverse organizations, agencies and sectors statewide and have a combined experience of over 21 years supporting our neighbors and communities in addressing the social determinants of health to ensure that all have equitable access to reach their full health potential.
Meet the team that make up our Coordinating Council
Maite Aros
Chair
As the Domestic Violence Community Health Advocate Program Coordinator at CHILD, Inc., Maite leads a groundbreaking collaborative pilot project uniting multiple healthcare institutions including CHILD, Inc., DCADV, Christiana Care, Westside Family Health, and St. Frances Hospital. This initiative uniquely approaches domestic violence as a public health issue, recognizing its impact across all socioeconomic, racial/ethnic, and gender backgrounds. With nearly three decades of experience in domestic violence services, Maite brings comprehensive expertise to her role, supported by a five-year bachelor’s degree in Social Work from the Pontifical Catholic University of Chile.
Her career encompasses extensive work as a domestic violence shelter manager, bilingual hotline counselor, case manager, and advocate, demonstrating her commitment to serving underserved populations. She further extends her impact as a group facilitator for the Spanish-Speaking Domestic Violence Counseling Intervention Program and as a Parenting Education instructor. Her excellence in the field has been recognized with the 2018 Elaine Aviola Memorial Award for Outstanding Professional, and she holds a DCADV Domestic Violence Specialist certification from 2006.
Delsy Morales
Vice-Chair
Delsy brings over a decade of experience in community health, program coordination, and advocacy across Delaware. With a strong background in managing multi-level health programs, Delsy has led initiatives supporting maternal health, youth development, and health equity. Delsy is currently working as a Program Manager at Impact Life advocating for Recovery Friendly Workplaces and supporting the Homeless Diversion Program.
Her passion for community empowerment is matched by her ability to manage staff, oversee grants, and coordinate large-scale events and educational outreach. As a bilingual leader and lifelong community advocate, Delsy has served on numerous boards and task forces, including the Maternal Mortality Review Board and the Community Health Worker Association of Delaware. She currently chairs the Wilmington Hispanic Parade with Nuestras Raices Delaware.
Delsy holds a Bachelor of Science in Human Services from Springfield College and brings a vibrant combination of lived experience, professional expertise, and heartfelt commitment to every program she touches. We are honored to have her leadership on the team.
Fun fact: Delsy was the Northeast Regional Youth of the Year in 2010 for the Boys & Girls Club and walked in the Oval Office with Denzel Washington to meet former President Barack Obama.
Nivia Brown
Secretary
Nivia serves as the current Secretary for the Association, where she ensures the organization operates with transparency and integrity. With a professional background in Statistics, data analysis and community engagement, Nivia brings over 15 years of experience in strategic planning, and rural health advocacy to the board. Nivia is deeply committed to expanding rural health access and partnering with Delaware leaders to expand the reach of CHWs into all industries, ensuring every community member has a trusted advocate to help navigate life’s challenges, beyond the walls of a clinic. When not managing records, or keeping track of meeting minutes, Nivia can be found enjoying free time with her family.Â
Erin Caple
Treasurer
Erin Caple is the Technical Assistance Manager at the Center for envision & the Center for Community Health Alignment (CCHA), where she leads national efforts to strengthen and sustain the Community Health Worker (CHW) workforce through training, technical assistance, resource creation and capacity-building. She has worked with communities in Delaware for several years in different spaces, bringing experience in family services, health education, and public health. Erin is passionate about creating interactive learning experiences that equip CHWs with practical tools, while also uplifting their voices and leadership. She is recognized nationally for her work advancing CHW integration, sustainability, and workforce development, and continues to center community voices in every project she leads.
Erin Ridout
Advocacy Committee Chair
Erin Ridout works in ChristianaCare’s Community Health Impact Department, supporting health equity and community health initiatives within Women’s Health. Prior to working at ChristianaCare, Erin led the Delaware Coalition Against Domestic Violence (DCADV)’s public health response to domestic violence. Erin has also worked as a social worker in the fields of domestic violence, therapeutic services, and child welfare. Additionally, she serves on the Coordinating Council for the Community Health Workers Association of Delaware (CHWAD), chairing the Advocacy Committee’s efforts for CHW certification and sustainable funding.
Mignaliz Nanny Vega-Morales
Training Committee Chair
Born in Ponce, Puerto Rico, Nanny Vega-Morales is a Bilingual Survivorship Specialist and Cancer Patient Advocate at the Delaware Breast Cancer Coalition (DBCC), with over 20 years of experience in healthcare. She holds a BA from the University of Puerto Rico, Mayagüez Campus, and an MBA from the University of Phoenix. She is also a certified interpreter, Health Coach, and International Life Coach specializing in Resilience.
Nanny is dedicated to improving access to care for the Hispanic community, serving as a critical bridge between patients and essential health resources. She walks alongside patients at the beginning of their diagnosis journey, helping them navigate complex systems with clarity and support. Her work focuses on addressing barriers, promoting equity, and ensuring culturally responsive care for cancer survivors.
As a three-time cancer survivor, she brings a powerful personal perspective to her advocacy, strengthening her commitment to connect individuals with the resources, opportunities, and support they need to navigate their journey with dignity.
Denise Ortiz
Resource Committee Chair
Denise Ortiz has been a Community Health Worker since 2022 and earned her Bachelor’s Degree from Wilmington University in 2025, where she was recognized as an Alumni Spotlight for breaking barriers as a single mother and creating a path for success. She currently works as a Navigational Coach at The WRK Group.
With a background working alongside Delaware Public Health, Denise has gained valuable experience supporting community-based health initiatives and addressing the needs of underserved populations. She connects clients with essential health and social services and helps them overcome barriers that make accessing care and resources challenging. Supporting Spanish-speaking clients who face difficulties with English is an added strength she brings to her work.
In 2024, Denise was also a finalist nominee for the CHW Champion Award, highlighting her dedication and impact in the field of community health work.
She serves through case management support, conducts outreach, and assists with resource coordination such as housing, food, and employment services, while building strong, trusting relationships within the community. Denise is passionate about advocating for underserved communities, helping individuals navigate complex healthcare and social service systems, and empowering clients to take control of their health and well-being. Her work focuses on improving access, promoting equity, and ensuring that every individual she serves has the tools and support needed to thrive.
Looking ahead, Denise plans to attend law school with the goal of helping those in need gain access to fair and equitable legal support.
Ama Amponsah
Co-Chair of Communications
Ama Amponsah is the Domestic Violence and Community Health Program Manager for the Delaware Coalition Against Domestic Violence (DCADV). Ama collaborates with member programs and community partners in this position to address and improve the public health response to domestic violence and engages in policy advocacy around Community Health Worker sustainability in Delaware. Ama’s work has included providing training and resources to different healthcare providers and their support staff, coordinating statewide efforts to build awareness of DV, the available services, trauma-informed screening practices, and supporting project evaluation and continuous quality improvement efforts. Ama also works with the Training and Outreach Coordinator, assisting with planning, implementing, and presenting domestic violence trainings, activities, and conferences to advocates, survivors, social service providers, law enforcement, and community members. Ama holds a B.A. in Anthropology from Washington College.Â
Monique McMillian
Co-Chair of Communications
Monique Mcmillian is a Delaware native and Certified Community Health Worker who now resides in Cecil County, MD. She serves as Co-Chair of Social media & Communications for the association and is a dedicated CHW at ChristianaCare, where she plays a key role in supporting patients and families through care coordination, resource navigation, health education, and advocacy. Her work centers on helping individuals overcome barriers and stay connected to the services that support their well-being.
Monique’s career is grounded in more than a decade of hands-on experience, beginning as a caregiver for individuals with developmental disabilities in Cecil County, MD and in 2018 becoming a formally trained CHW through the University of Maryland Upper Chesapeake health system. This early work shaped her commitment to person-centered support, dignity, and equity. Her CHW practice reflects that foundation, with strengths in benefits navigation, chronic disease support, and building trusted relationships with diverse communities.
In addition to her work at ChristianaCare, Monique is the founder of In the Gap Care, a mobile senior support initiative that provides non-clinical wellness check-ins, companionship, and practical assistance. Across all her roles, she is dedicated to meeting people where they are and ensuring every individual feels seen, supported, and empowered.
Adriana Viveros-Sosa
At Large Member
Adriana Viveros-Sosa was born in Jalapa, Veracruz, Mexico, and came to the United States at nine years old. As a DACA recipient, interpreter for her parents, and first-generation graduate of Kennett High School, Adriana learned early on the power of advocacy and the importance of giving a voice to those who cannot speak for themselves.
She now serves as the Community Health Worker Liaison for the Delaware Breast Cancer Coalition, connecting individuals to critical health resources and overseeing multilingual patient navigation services across the state. Adriana is also a member at large for the Community Health Worker Association, supporting efforts for frontline workers with lived experience.
Her passion for community empowerment extends through her roles as a board member for AIDS Delaware and a consultant for the Hispanic Association of Delaware, where she helps educate Spanish-speaking families about child abuse prevention and immigrant rights.
Rooted in empathy, maternal health advocacy, and social justice, Adriana continues to build bridges for underserved communities—always guided by her love for her four children and her unwavering commitment to care, equity, and empowerment.
