Community Health Workers (CHWs) are vital bridges between healthcare systems and the people they serve. In Delaware, CHWs have reduced hospital admissions by 19% and improved follow-up care compliance by 22% in vulnerable communities. Yet, many people don’t know what CHWs do or how they can help.

That’s why we’re launching our new CHW Ambassadors Program!

Our team of volunteer CHW Ambassadors are ready to visit your organization to share how CHWs can transform community health outcomes and save healthcare costs—with proven returns of $3.77 for every $1 invested.

What are Community Health Workers?

CHWs are trusted community members who connect people with essential resources and improve health outcomes by:

  • Providing health education on prevention, chronic condition management, and healthy lifestyles
  • Helping navigate complex healthcare systems, appointments, and insurance enrollment
  • Conducting outreach to underserved populations and delivering critical health services

Meet Our CHW Ambassadors

Adriana currently serves as the Lead for Resources at the Community Health Worker Association of Delaware, where she is committed to promoting the Association to all organizations and community members. She actively works to ensure that Community Health Workers (CHWs) are supported and equipped with the necessary tools and resources to serve their communities effectively. Adriana’s vision is to build strong partnerships that will elevate the work of CHWs and ensure that their invaluable contributions are recognized and celebrated across the state.

A champion for those who are often overlooked, Adriana’s mission is to give strength to individuals who want to be seen and heard. She works tirelessly to ensure that no one falls through the cracks. “I give a voice to those who want to be heard,” she says, “and I will move mountains to make sure everyone gets the same information and resources.” She is passionate about creating a lasting memento of the power and impact of community health work, and deeply believes in the importance of the CHW Association of Delaware.

Adriana also serves as the Community Health Worker Liaison at the Delaware Breast Cancer Coalition, where she provides vital wrap-around services, health literacy support, and advocacy for individuals facing breast cancer. In this role, Adriana attends doctor’s appointments with patients, assists with healthcare navigation, and ensures that individuals receive the care and compassion they deserve during difficult times.

Additionally, Adriana has worked as a Program Coordinator for Christiana Care’s Health Ambassador Program, where she used her life experience to help bridge gaps in healthy lifestyle access. As a board member for AIDS Delaware, she continues to advocate for social justice, health equity, and the needs of underserved communities.

A DACA recipient, Adriana is no stranger to the barriers faced by immigrants in the United States. She has channeled her personal experiences into a lifelong mission to break down those barriers for others, advocating for systemic change in healthcare and beyond. Her work is deeply rooted in maternal health and social determinants of health, with a special focus on ensuring that everyone—regardless of background—has access to the resources they need to thrive.

Above all, Adriana’s most cherished role is as a mother, and she strives every day to be a positive role model for her children, showing them the importance of hard work, compassion, and advocacy.

Indi Islam

Indi Islam is a dedicated Community Health Worker, certified Health literacy specialist, and a Community Engagement Specialist. With a strong foundation in health education and outreach, I am passionate about empowering individuals through accessible health information, currently working in the field of HIV prevention.

Lillian Reyes

Lillian Reyes

Lillian Reyes is a Community Health Worker Ambassador (CHW), Certified Clinical Medical Assistant (CCMA), Certified Phlebotomy Technician (CPT), Certified EKG Technician (CEKGT), Certified Mental Health First Aider, Certified Bilingual Interpreter (Spanish), Diabetes Lifestyle Coach, and Notary for the State of Delaware. She is a dedicated bilingual Community Health Worker with deep roots in the Latino community, passionate about improving health access and outcomes for underserved populations.

With three years of experience as a community health worker and more than seventeen years as a medical assistant, Lillian actively connects individuals to vital healthcare services, provides culturally competent health education, and advocates for community needs, particularly focusing on specific health concerns such as diabetes management, congestive heart failure, and high blood pressure. As an ambassador, Lillian enjoys providing education, outreach, and navigation, bridging the gap between communities and the healthcare system to ensure that everyone has access to the resources and support they need to thrive.

Maia Michael

Maia Michael is a dedicated Community Health Worker with Delaware First Health and a proud native of Delaware. Through her work in the community, Maia conducts home visits and serves as a vital connector—linking individuals and families to essential healthcare services and resources across the state. Passionate about improving access and equity in healthcare, she brings both local insight and heartfelt commitment to her role as an ambassador with the Community Health Worker Association of Delaware.

When I’m not out changing lives, you might find me cruising on a bike, “playing” tennis (rules optional…..lol), being a doula or soaking up the sun.

I am a CHW Ambassador because our people fail for lack of information. While I can’t be everywhere, helping everyone, I can share what I know with who I know. I can inform and empower others to go out and help heal their slice of the world.
