Careers and Volunteers
CHWs are a valued part of the health care team. They serve as frontline liaisons, guiding individuals and families through the health, social, and community services systems to achieve better health and well-being.
CHWs support communities throughout Delaware, working with hospitals, Federally Qualified Health Clinics, the Division of Public Health, community organizations, and managed care organizations. CHWs are part of an equity-focused public health approach to support individuals and communities based on their health-related social needs.
